What is an Elder Care Manager?
An elder care manager is a health and human services professional such as a nurse or social worker with a specialized focus on issues related to aging and elder care. At One Senior Place, visitors are invited to meet with a Care Manager for a preliminary consultation. There is no charge, and family members are encouraged to attend.

In many cases, the senior services and products you need will be available from independent businesses and providers located right here at One Senior Place. If you wish, the Elder Care Manager can introduce you or make an appointment on your behalf.
Some seniors may benefit from extended care management services, for example, a couple who had been independent until the husband lost his driver license, or a single lady with mild dementia and no family nearby who wants to continue living at home. In such cases, expanded care management services can be invaluable. When additional services are requested, the Elder Care Manager will schedule a comprehensive assessment, then work with the older adult, their family, and other members of the “care team” as appropriate – doctors, attorney, trust officer, care and service providers – to create a plan of care that meets the needs of the elderly client. An Elder Care Manager will meet with you to help you understand your loved one's needs and to learn what resources and options are available to meet those needs.
How much does it cost?
At One Senior Place, there is no charge for your initial consultation with an Elder Care Manager. For many clients, the included recommendation and referrals will be sufficient. For those with more complex needs, a variety of expanded services are available. Those services are always optional, and the Care Manager will clearly explain all available services and associated costs.

We welcome you to visit us at our Viera location, or you may contact us by phone or email. The client and interested family members may meet with a One Senior Place Elder Care Manager in our private Resource Library Conference Room, or if necessary, the Care Manager will come to you at your home or care facility. For families at a distance, the consultation can be done over the phone.
